Patient Centered Care Partners was founded by Patricia Stern, LCSW, MBA, is a clinician with an extensive background in behavioral health and primary care delivery. Ms. Stern’s experience includes private practice of psychotherapy, managing  a hospice; creating the first remote department  of social services in a chronic care company; creating a department of Care Champions for an insurance company; liaising to home health care in a hospital system and providing medical social work home visits. Ms. Stern provides therapy in her private practice in Northbrook as well as online. She regularly teaches clients how to be more powerful in communications using her original phrase "no excuses, no reasons, no stories". Her experiences have shown her where patients can be helped and where systems can be made more efficient.
